Promotion rules:

When scheduling an appointment for a New Patient exam, Promotion must be mentioned and appointment must be scheduled and kept between January 5, 2018, and Feb 28, 2018, with either Dr. Jennifer Place, Dr. Sally Hoey, Dr. Sandy DiPonio, or Dr. Paul Feinberg.

This Promotion does not apply to those who have scheduled a New Patient appointment prior to January 5, 2018, for any time between January 5, 2018, and February 28, 2018.

This Promotion applies if mentioned during the initial scheduling of the New Patient appointment. This Promotion does not apply once the appointment has been made.

This Promotion applies if a New Patient appointment is canceled and then rescheduled only if the Promotion was applied to the first appointment and the new appointment is scheduled and kept before February 28, 2018.

If the patient is using medical insurance, the exam discount does not apply.

If the patient is using vision insurance, the frame and lens discounts do not apply.

Doctors and other healthcare providers and case managers are not eligible for the referral award in this promotion.

Patient is entitled to only 1 promotional discount.

Referrers will receive a referral award for each New Patient examination that is scheduled and kept between January 5, 2018, and February 28, 2018, and adheres to the other rules.

This promotion cannot be applied to any co-pays or deductibles.
